  • One Initial Advance Care Planning Session where we go through the ACD together or come up with an alternative ACP that addresses your wishes. This usually takes around 2 hours but could be longer (or shorter) depending on your needs. I will take you step-by-step through the ACD, review your ACD, and provide you with researched responses that can assist you in making your choices. Virtual or in person.

  • One Follow-up ACP Call If the completion of your ACD is not urgent, this session can be about 2 weeks after our initial session and gives you time to reflect on any additional questions or clarifications, and for me to provide you with any additional information you may need to complete your ACD. Includes a final review of ACD. Virtual; scheduled at the end of our initial planning session.

  • Email support should you have questions about ACP between your initial session and follow-up call.

  • ACP Step-by-Step Introduction: An easy-to-read cheat sheet that outlines the basic steps of Advance Care Planning & Advance Directives, as well as a checklist for ACP.

  • Comprehensive 30-page Advance Care Directive: This ACD includes sections you are welcome to fill out or skip and is made with the intention to offer customisation for your wishes to be recorded. includes Medical Power of Attorney, healthcare instructions, living will & funeral planning.

  • ACP Resource Presentation: A 45-minute presentation on the essentials of Advance Care Planning. Includes helpful Victoria-specific ACP tips and links.

Should you wish to book this package with a group of two or more individuals, there will be a fee of $200. Please contact Dina directly to arrange group bookings and payment. There is a 5 person maximum to preserve the intimacy of the session.
